Without Referral<\/strong><\/h5>

I am affiliated with AVIG (Association of Integrative Medicine Physicians). As a result, consultations are often partially covered by supplementary health insurance. Check with your health insurer how much of the consultations will be reimbursed. <\/p>

If you don't have supplementary insurance, you must pay the full invoice yourself. This can be done directly at the practice location or later via invoice. <\/p>

Additional tests are generally not covered. Non-claimable costs may be tax-deductible. <\/p>

With Referral from GP, Company Doctor, or Specialist<\/strong><\/h5>

If you have a referral from your GP, company doctor, or medical specialist, the care I provide may fall under specialist medical care. This would then be covered by basic insurance. <\/p>

However, the practice has no contracts with health insurers. For small healthcare providers, it is unfortunately difficult to establish contracts with health insurers. <\/p>

Nevertheless, the provided care is covered by basic insurance. This is because in the Netherlands, we have the right to choose our own doctor. Depending on your policy, your health insurer reimburses 65-100% of healthcare costs from non-contracted providers. The percentage of healthcare costs that is not reimbursed will be covered by the practice (courtesy arrangement). You will then receive a discount on the invoice. <\/p>

Deductible<\/h5>

You should always take into account the deductible you have agreed to.<\/p>

The deductible is set annually by the government and no discount may be given on this. For 2025, the mandatory deductible is set at \u20ac385. Terms and Conditions<\/h5>

This document contains the general terms and conditions of the Practice for Integrative Immunology (hereinafter: 'the Practice'). These conditions apply to all agreements and treatment relationships between the Practice and clients\/patients. <\/p>

Article 1. Definitions and Terms <\/h5>

In these general terms and conditions:
- Practice: Practice for Integrative Immunology, including supporting staff and\/or third parties engaged by the Practice.
- Client\/Patient: the counterparty of the Practice when entering into a treatment agreement.
- Treatment Agreement: the agreement within the meaning of the WGBO between the Practice and the client\/patient.
- Medical Treatment: consultations, medical treatments or non-conventional treatment methods, diagnostics and other services.
- Informed consent: the information-based consent of the patient for treatment.
- Wkkgz: Healthcare Quality, Complaints and Disputes Act.
- AVG: General Data Protection Regulation
- WGBO: Medical Treatment Agreement Act<\/p>

Article 2. Applicability <\/h5>

These conditions apply to all agreements between the Practice and the client\/patient.
Deviations are only valid if agreed in writing.
If provisions are invalid, the remaining provisions remain in force.<\/p>

Article 3. Treatment <\/h5>

The treatment agreement is entered into for the duration of the treatment, a trajectory or single intervention.
The Practice endeavors to provide care according to current professional standards and state of science. For non-conventional treatments, the KNMG guideline 'The physician and non-conventional treatments' dated May 26, 2022 is followed. This is a best-efforts obligation, not a results obligation.
The client\/patient provides all necessary and correct information in a timely manner and follows the Practice's advice.
The Practice may engage third parties. Articles 7:404, 7:407 paragraph 2 and 7:409 of the Dutch Civil Code are excluded. <\/p>

Article 4. Rates and Payment <\/h5>

The rates are based on the Practice's most recent price list available at www.pvii.nl
Payment must be made:
- Immediately after the consultation, or
- Within 28 days of invoice date, via bank transfer or iDEAL.
If the patient has not paid the invoice within the specified payment term, the practice will send the patient a notice of default in the form of a payment reminder. The patient has the opportunity to pay the invoice within 14 days of the date of the payment reminder.
After exceeding the second payment term, the client\/patient is in default and statutory interest is due. Collection costs are at the expense of the client\/patient. <\/p>

Article 5. Cancellation of Appointments <\/h5>

Appointments must be cancelled at least 24 hours in advance.
For late or no cancellation, the Practice may charge the consultation costs (usually the full rate, depending on type of consultation).<\/p>

Article 6. Insurance Reimbursement <\/h5>

It is the client\/patient's responsibility to check in advance whether the treatment qualifies for reimbursement by the health insurer. The Practice is not responsible for rejection of claims. <\/p>

Article 7. Privacy and Medical Records <\/h5>

The Practice processes personal and medical data in accordance with AVG and WGBO.
Medical records are kept for 15 years, unless another term applies.
The client\/patient can access their records, request a copy, or submit requests for destruction.
If the data is used - anonymously - for scientific research or statistics (provided this is within legal frameworks), this will always be discussed in advance by the Practice and will be documented in writing between the Practice and the client\/patient.<\/p>

Article 8. Liability <\/h5>

The Practice is not liable for damage resulting from incorrect or incomplete information from the client\/patient or failure to follow advice.
The Practice's liability is limited to the amount covered by liability insurance, except in cases of intent or gross negligence.
The Practice is not liable for the quality of supplements or medications supplied by third parties.<\/p>

Article 9. Termination of Agreement <\/h5>

The agreement can be terminated by both parties:
- By the client\/patient with written notice.
- By the Practice for compelling reasons, such as damaged trust relationship, repeated failure to keep appointments, or non-payment.
Upon termination, obligations regarding record-keeping and payment remain in effect.<\/p>

Article 10. Code of Conduct <\/h5>

Rules of respect, safety, and privacy apply within the Practice. Aggression, discrimination, and (sexual) harassment are not tolerated. <\/p>

Article 11. Complaints and Disputes <\/h5>

The Practice has a complaints procedure in accordance with the Wkkgz.
Complaints are preferably first discussed in a personal conversation. If this does not provide a solution, the client\/patient can contact an independent complaints officer of the AVIG
Dutch law exclusively applies to all legal relationships. Parties will endeavor to resolve disputes through consultation before going to court. <\/p>

Article 12. Changes to Terms and Conditions <\/h5>

The Practice reserves the right to modify these terms and conditions. Changes will be announced to clients\/patients in a timely and clear manner. <\/p><\/div> <\/div>\n\t\t\t\t\t\t

Privacy Policy<\/h5>

This privacy policy has been drawn up by the Practice for Integrative Immunology (hereinafter: 'the Practice'). We respect your privacy and handle your personal data with care. This statement has been drawn up in accordance with the General Data Protection Regulation (GDPR) and the Medical Treatment Agreement Act (WGBO). <\/p>

Article 1. Purpose and Legal Basis for Data Processing <\/h5>

The Practice processes personal data solely for healthcare provision, administration, legal obligations, and \u2013 if applicable \u2013 scientific research, education, or information.
The legal basis for this is:
- Performance of the treatment agreement (GDPR Art. 6 para 1 sub b);
- Legal obligations, such as notifications under the Public Health Act;
- Legitimate interest, such as quality improvement of care;
- Consent of the client\/patient, for example, when exchanging data with third parties.<\/p>

Article 2. Types of Personal Data Processed <\/h5>

Depending on the treatment and services, the following data may be processed:
- Name, address, place of residence;
- Date of birth, gender;
- Citizen Service Number (BSN);
- Phone number, email address;
- Medical data and treatment data;
- Insurance details;
- Data of general practitioner and other involved healthcare providers;<\/p>

Article 3. Purposes of Processing <\/h5>

The personal data are processed for:
- Creating and maintaining an (electronic) patient file;
- Scheduling appointments and carrying out treatments;
- Financial administration and invoicing;
- Communication with referrers and other healthcare providers (with consent);
- Complying with legal obligations;
- Quality improvement and scientific research (anonymized and only with consent).<\/p>

Article 4. Retention Periods <\/h5>

Medical data is generally retained for 15 years from the last treatment, unless a longer retention period is necessary due to the patient's health or legal obligations.
For minors, a retention period applies from the age of 18.<\/p>

Article 5. Disclosure of Data to Third Parties <\/h5>

Personal data is only provided to third parties:
- If this is necessary for the treatment;
- If a legal obligation requires it;
- If the client\/patient has given explicit consent for this.
Data processing agreements are concluded with external processors.<\/p>

Article 6. Rights of the Client\/Patient <\/h5>

You have the following rights:
- Right to access your data;
- Right to rectification or completion;
- Right to erasure or destruction (insofar as legally permitted);
- Right to data portability (transfer of your file);
- Right to restriction of processing;
- Right to object to processing;
- Right to add your own statement to your file.<\/p>

Article 7. Security of Personal Data <\/h5>

The Practice takes appropriate technical and organizational measures to secure your data against loss, unauthorized access, or unlawful processing. Examples include the use of secure systems and professional secrecy of employees. <\/p>

Article 8. Digital Storage and Website <\/h5>

Your data is stored digitally in a secure system. The electronic patient file used is NEN7510 and ISO27001 certified. If a website with a contact form or cookies is used, personal data is processed solely for answering questions and scheduling appointments. <\/p>

Article 9. Complaints and Oversight <\/h5>

If you have questions or complaints about the processing of your data, you can contact the Practice.
If your complaint is not handled to your satisfaction, you can contact the Dutch Data Protection Authority (www.autoriteitpersoonsgegevens.nl).<\/p>

Article 10. Amendments to this Privacy Policy <\/h5>

The Practice reserves the right to amend this privacy policy. Amendments will be announced to clients\/patients via the website or in writing. <\/p><\/div> <\/div>\n\t\t\t\t\t\t

Complaints Procedure<\/h5>

I highly value providing expert care and fostering a good treatment relationship with you. However, it may happen that you are not satisfied or have a complaint. I would prefer you to share this with me. By discussing your complaint, we can work together to find a solution and I can further improve the quality of my care. <\/p>

Complaints Officer<\/h5>

If we cannot reach a solution together, you can submit your complaint to an independent complaints officer. The practice is affiliated with the Artsen Vereniging Integrale Geneeskunde (AVIG) for this purpose. The complaints officer will listen to your complaint, explain the options, and try to reach a solution with you and me. <\/p>

Disputes Committee<\/h5>

If mediation also does not offer a solution, the complaints officer can support you with a next step.
